Privacy statement

I collect personal information from clients, including information relating to:

  • name
  • contact information
  • billing or purchase information
  • personal information, including but not limited to: medical and psychiatric histories, family and individual histories, and clientr beliefs, thoughts, and emotions.

This personal information is used to provide counselling services.

The confidentiality of information provided for counselling is recognised in law. Nevertheless, when I believe it necessary, I may need to share this information with relevant third-party agencies to protect clients and others from significant risk of harm. Anonymous case information may also be shared with clinical supervisors. In the event client information is subpoenaed by a court, whenever practicable, I will challenge this – as provided for by my professional indemnity insurance.

Providing some information is optional. However, if clients choose not to provide information I believe necessary to provide the service, I will be unable to provide or continue to provide counselling.

I keep client information safe by either storing it securely or holding it in encrypted electronic form, and only allowing approved staff to see it.

I keep client information for 10 years, as required by the New Zealand Association of Counsellors, at which point I securely destroy it by shredding all paper documents and erasing all digital information.

Clients have the right to ask for a copy of any personal information I hold about them, and to ask for it to be corrected if they think it is wrong. If clients wish to have copies of their information, or to have it corrected, please contact me at enquiries@montem.com, or 027 551 5002. I may require prior payment of all reasonable costs in providing these copies.

If clients are concerned with any aspect of my holding their information, please contact me in the first instance and I will endeavour to resolve any issues. In the event clients remain dissatisfied, they may contact the New Zealand Privacy Commissioner on 0800 803 909 (Monday to Friday, 10:00 am to 3:00 pm); email investigations@privacy.org.nz; write to PO Box 10 094, The Terrace, Wellington 6143; or visit Level 13, 51–53 Shortland Street, Auckland.
